What we collect

- Health information from vaccination consent forms (medical history, allergies, medication).
- Technical data when you browse this website.
How we use it
- To provide workplace vaccination services and respond to enquiries.
- To clinically screen employees before vaccination.
- To meet regulatory obligations (GPhC, MHRA).
- To report to employers, uptake numbers only; the standard flu-clinic report does not include individual medical histories or screening answers. Any necessary individual occupational health reporting requires appropriate confidentiality and data-protection arrangements.
Your rights
You can ask for access to or correction of your personal information, and raise a request about deletion, restriction or objection. Website enquiries
The quote form sends business contact and booking information to HYL Health using EmailJS. Do not use it to submit employee medical information. The form asks where you heard about us to help us understand enquiries; that field is optional. See EmailJS’s privacy information.
Clinical information and retention
Clinical information is collected by the pharmacy team for screening, care and professional record keeping. Retention depends on the record’s purpose and applicable clinical, regulatory and legal requirements. Contact us for the retention arrangements applicable to your enquiry or clinical record.
